Hypnotism authorisation
If you wish to show, demonstrate or perform a hypnotism act (as defined in the Hypnotism Act 1952) on any person in public within the borough, you must have written authorisation from the council.
Hypnotism for the purpose of this application includes hypnotism, mesmerism and any similar act or process which produces or is intended to produce in any person any form of induced sleep or trance in which the susceptibility of the mind of that person to suggestion or direction is increased or intended to be increased, but does not include hypnotism, mesmerism or any such similar act or process which is self induced.
Upon completion of your application you will be required to confirm that you:
- Have read fully and understood the contents of this form and any supporting information and agree to the terms and/or conditions set out within
- Understand that by submitting this application, you consent to High Peak Borough Council passing on the data within this application and its supporting documents to any authority or person that will assist its determination of this application, or as required by law
- Declare that the information provided is true to the best of your knowledge and belief
- Understand that if any false information is provided you may be guilty of an offence and liable to prosecution
- Understand that if the application is authorised you must inform the authority about any changes to circumstances that mean you no longer meet the conditions for the authorisation
- Understand where a statutory declaration has been provided it may be forwarded to the police
